脑积水是神经外科的一种常见疾病,感染、畸形等多种病因可诱发脑积水,脑积水的日常管理占儿科神经外科医生工作量的近一半。脑积水儿童中枢神经系统(CNS)肿瘤的流行病学分布与成人不同。儿童后颅窝肿瘤(PFT)的比例很高,脑积水通常是第一表现,有些病例在手术后出现新发或加重的脑积水。关注脑积水,关注儿童后颅窝肿瘤非常具有现实意义。

我们的文章综述了文献的最新进展和临床实践中遇到的实际问题。对大多数儿童来说,手术引流是最有效的治疗方法;然而,可以在手术或做出明确诊断之前进行药物治疗。PFT是儿童中枢神经系统肿瘤的常见部位,其临床表现常与脑积水有关。在PFT患儿中,可以根据患儿的病情初步预测脑积水的风险,早期预测脑积水风险对治疗方法的选择和相关预后的评估具有明确的临床意义。

依达拉奉右莰醇由依达拉奉和右莰醇构成,是一种新型神经保护剂,虽然依达拉奉-右莰醇的疗效已通过先前的随机对照试验(RCT)确定,但缺乏在现实世界实践环境中治疗有效性和安全性的证据。

先前的临床试验主要集中在AIS的早期(症状出现后48小时内)和未接受再灌注治疗的患者,这与临床实践不同。先前临床试验的样本量相对较小,在随机对照试验和真实世界人群之间,具有脆弱特征的个体的代表性极为缺乏。

因此,EXPAND旨在确认依达拉奉右莰醇在现实世界中AIS患者中的作用。EXPAND是全球首个真实世界中探索依达拉奉右莰醇治疗急性脑梗死安全性和有效性的研究,这项研究将阐明依达拉奉右莰醇在真实世界实践中对AIS患者症状出现后14天内的有效性和安全性,我们的研究样本量大,与以前的随机对照试验相比,入组标准要宽松,这使得延长神经保护治疗的治疗时间窗口成为可能,并提出了针对AIS的多靶点研究方案。

The progression of surgical intervention for arterial reocclusion after mechanical thrombectomy for acute cerebral infarction

Acute ischemic stroke, a prevalent cerebrovascular disease, significantly contributes to mortality and morbidity in modern society. Although early mechanical thrombectomy is beneficial for many patients, a subset still suffers from failed recanalization due to arterial occlusion (AR) post-thrombectomy, owing to a variety of factors. Therefore, minimizing vascular reocclusion rates is imperative in improving outcomes for patients with acute cerebral infarction. Recent surgical intervention studies have concentrated on vascular recanalization after mechanical thrombectomy. This review critically analyzes the pathophysiology and effective treatments for vascular reocclusion, providing insight into the latest developments and outlining challenges for future research.

The progression of artificial intelligence technology and Parkinson’s disease

Parkinson’s disease is a neurodegenerative disease that seriously endangers the health of middle-aged and old people and is characterized by the degeneration of nigrostriatal dopaminergic neurons as its main pathologic feature. Due to its numerous influencing factors, unclear pathogenic mechanisms, and complex clinical manifestations, the diagnosis and treatment of Parkinson’s disease still face huge challenges. In recent years, artificial intelligence technology has developed rapidly and its application in the medical field has become increasingly widespread. This article reviews the achievements of artificial intelligence in the diagnosis and treatment of Parkinson’s disease, with a view to benefiting patients with Parkinson’s disease in the future.

Hydrocephalus is a common disease in neurosurgery, and its daily management accounts for nearly half of the workload of pediatric neurosurgeons. The epidemiological distribution of pediatric central nervous system (CNS) tumors differs from that of adults. The proportion of posterior fossa tumors (PFTs) in children is high, hydrocephalus is often the first manifestation, and some cases have denovo or aggravated hydrocephalus after surgery. However, controversies remain regarding the best treatment methods and timelines for PFT with hydrocephalus. This article reviews the latest progress in the literature and practical problems encountered in clinical practices.

Edaravone dexborneol has been confirmed as a novel neuroprotective agent by acting on multiple active targets of antioxidation and anti-inflammation. This study aims to evaluate the effectiveness and safety of edaravone dexborneol in patients with acute ischemic stroke (AIS) within 14 days of symptom onset in real-world clinical practice.
